Referrals, Reporting and Quality Management

Urgent Referrals

All urgent referrals must be made by telephone to ensure that the necessary action is taken immediately.

Advance Referrals

Telephone bookings are accepted, however if you prefer to send us your bookings electronically, please kindly contact us for our Booking Request Form. Confirmation of your booking will be provided.

After Hours Referrals

All referrals after hours (including weekends) must be made by telephone to ensure receipt.

Reporting

A standard report sheet may be completed if required and provided to the service user following each referral. The report sheet will contain details of events or incidents which took place whilst the client was in our care.

Quality Management

A quality control questionnaire may be sent to the service user occasionally. We would appreciate it if you could complete and return this questionnaire to us as it will enable us to monitor our performance and improve our service.
